Transportation Management Associations (TMA)

Transportation Management Associations (TMAs) are non-profit, public-private partnerships that help improve mobility. They offer accessible transportation solutions, including transit information, community shuttles, and van/carpool programs.

ModivCare – Request A Ride To A Medical Appointment

If you have NJ FamilyCare, you can request a ride from ModivCare for non-emergency medical appointments anywhere in New Jersey.

To schedule your ride, call 1-866-527-9933 two days before your medical appointment. Book your trip online: tripcare.modivcare.com/login.

You will need to provide the following information:

● Your NJ FamilyCare ID number

● Your pick-up address and zip code

● Name, phone number, and address of medical provider

● Time and date of your medical appointment

● Any special transportation needs
